Mark gave a talk on the 'Swiss Cheese Model' of risk.  There are some roles in this world where risk can not be excluded, the Swiss cheese model aims to put as many barriers between a good day in the office/plant and a bad day - while giving operators as much information to make balanced judgments.  

Process Safety is the most important thing in any management of risk.
